55+ Service Navigator Program
As we navigate the COVID-19 pandemic, the 55+ Service Navigator Program is being offered via telephone. For assistance, please call (614) 559-0203 or (614) 559-0379.
Individual service navigation is the function of linking people, and their family members, with essential health, social, and community services. Service navigation may also include assistance with finding and scheduling transportation, as well as arranging for the provision of non-medical services such as housing, and social/emotional supports for the client and their families offered by other local social service agencies. The intent of service navigation is to assist people through complex health and social service systems. Service Navigation helps individuals overcome barriers in accessing quality care and treatment and increases their engagement and active participation in their care.
