bexley Social Services Program
BEACON: Bexley Engagement, Assistance & Community Outreach Network
BEACON is a social wellness program that aims to support Bexley residents in achieving stability, independence and overall well-being through compassionate engagement, linkage to appropriate community resources and short-term case management.
Residents, family members, or community members can refer an individual to the social services coordinator, a no-cost and voluntary service, by completing the referral form. The social services coordinator will contact the referred individual to complete an assessment and determine a plan of care. Care plans may involve providing education, referrals or other service coordination to best support the individual or family. The social service coordinator is not a treatment provider but will offer referrals to treatment providers as appropriate.
The social service coordinator can be reached Mondays, Tuesdays and Fridays during regular business hours at the Bexley City Hall offices.

A: The service coordinator acts as a bridge to the appropriate community resources. The service coordinator may complete an assessment with the referred individual to determine need and plan of care. Service coordinator may provide education, resources, referrals or short term case management to assist the individual/family.
A: Participation is voluntary. Individuals/families can decide to cease participation in the BEACON program at any time.
